NDIS Supports

Encouraging and building independence through care

  • Phoenix engages with young and adult NDIS participants across Victoria to support and empower them to achieve their goals. We believe that living with a disability should not limit a person’s potential and value equity and access for all.

  • Our qualified and experienced NDIS Support Workers support community-based and forensic participants in a range of key areas, including skills development, community participation and capacity building. Our services include:

  • Counselling
  • Support to attend appointments, education and employment
  • Transport
  • Developing daily living skills
  • Support to complete basic household tasks
  • Community access and outreach
  • Companionship and social support
  • Capacity building

Our booking process

Phoenix aims to make requesting supports easy. Follow the steps demonstrated to begin the booking process. Once you have spoken to someone from our Allocations team, we will redirect you to the webite to complete the below referral forms.
